Fee and Service Agreement — BLS Supervision Services
Our commitment to you
Your supervisor will have their Child Abuse Clearances and PA and State Criminal Clearances. Our supervision is "eyes on" at all times. If the supervisor believes you are being unsafe with the child or the child is in duress, the visit will be cancelled. Future visits may still occur. You are responsible to bring any toys, games, food, or entertainment for the children. You are responsible to provide the supervisor with a list of your child's allergies or health issues. The supervisor will not provide assistance during the visit and will not give medication.Conduct during visits
- No alcohol, drugs, or arriving under the influence of any substance.
- No physical discipline, threats, or yelling at the child.
- Appropriate language must be used at all times.
- The child is not to be questioned or discussed regarding the court case, custody matters, or the other parent.
- The child may not be photographed, recorded, or asked to keep secrets.
- There will be no whispering during visits.
- No phone use during the visit unless previously approved.
- No additional persons without prior court approval or approval from all parties.
- Parties shall not communicate with each other before or after visits.
Visit times / arrivals and departures
The parent being supervised shall arrive 10 minutes earlier than the visit time. They will have the opportunity to set up the room and prepare. The supervised parent shall leave after the children and the other parent have left the building.Gift giving and food
Gifts, food, or snacks may be brought only with prior approval by the supervisor or custodial parent. No items that are inappropriate, unsafe, or not age-appropriate. All items brought to the visit will be documented.Payment of fees
All fees are nonrefundable. You are paying for a supervisor to be available for the visits. Cancelled, rescheduled, or early-ended visits will not be refunded. Past due balances: 1.5%/month after 30 days · $20.00 collection fee at 60 days · $35.00 returned check fee.Costs incurred
You are responsible for any costs assumed by our company during your visit including cleaning, broken items, and travel expenses. All costs must be paid before the last visit can occur.Reports and testimony
A report is provided after each visit to the party who paid. Court testimony requires a subpoena at least 14 days prior. Cost: $100/hr · $200 minimum retainer due at least 14 days before the hearing.Recordings and photographs (18 Pa.C.S. § 5703)
By signing, you consent to recording of your voice, image, and likeness during visits. Recordings are used for reporting purposes only and shall not be disseminated to any non-employee or contractor of BLS. PA is a two-party consent state — this agreement satisfies that requirement.Use of technology and artificial intelligence
BLS utilizes AI tools to assist in the delivery of services. All AI-assisted work product is reviewed by a licensed attorney before being relied upon. BLS takes reasonable steps to protect client confidentiality and will use AI tools configured not to use client information to train external systems.Dispute resolution
All disputes shall be submitted to mandatory, binding arbitration in York County, Pennsylvania. You expressly waive your right to file a lawsuit in any civil court against BLS Supervision Services, the supervisor, staff, or contractors.Electronic signature

Fees
Rate: $120.00 per hour (1-hour visits). Mandatory intake fee: $100.00 per family unit — paid before intake meeting is scheduled. Three (3) visits must be prepaid ($360.00) before the first visit occurs. All fees are nonrefundable. Ongoing visits after the initial three must be purchased in pairs — minimum 2 visits ($240.00) per purchase. Court testimony: $100/hr · $200 minimum retainer due at least 14 days before the hearing.Mandatory intake
Prior to the first visit a mandatory intake meeting must be held between the supervisor and the visiting parent to review the child's needs, triggers, and goals. This does not obligate either party to continue with ongoing visits.Our commitment to you

The monitor provides a safe, neutral, conflict-free exchange of the child between parties and documents the interaction. The monitor does not take sides, make legal decisions, or interfere in custody disputes.Conduct during exchanges

- No discussion of legal matters, custody disputes, or court proceedings in front of the children.
- No derogatory comments about the other party in front of the children.

- Follow all direction from the monitor without argument.
Documentation

Fees
Intake Fee: $50.00 — paid via Stripe when scheduling intake meeting.Home Study Flat Fee: $750.00 — paid via LawPay (operating account). BLS will send you the LawPay payment link after your intake meeting. Your home visit will be scheduled once payment is received.
Testimony Retainer: $200.00 — $100/hr · minimum $200 due at least 14 days before testimony. If the fee is not paid, testimony will not take place. If scope expands beyond the original estimate, the firm reserves the right to request additional funds. You will be contacted before any additional fees are charged.
Process
Once payment is received, a visit to your home will be scheduled. All adult household members must be present. We will need access to all areas of the home and ask that dogs be crated if they may become territorial and/or aggressive. We will review the reason the home study has been requested prior to conducting the visit and will specifically examine any areas of concern raised by parties, counsel, or the Court. The finalized report will be distributed in accordance with the Court order. Fee includes: initial consultation/interview · review of docket information · visit to the home · contact with up to 2 adult references (if requested) · final report distributed per Court order. Pursuant to Pennsylvania R.C.P. 1915.8(b), if any party wishes to enter the final report into evidence, the author of the report must be subject to cross-examination.Information & Confidentiality
Email communications may not be privileged. We communicate only with the named client regarding case status — no updates to outside parties. Virtual meeting recording consent is granted by signing this agreement, valid until completion of your matter.NON-REPRESENTATION NOTICE: This is NOT an agreement for legal representation. Your relationship with SYDNEY CH BENSON, ESQUIRE LLC AND/OR BENSON LEGAL SERVICES is limited to the completion of a home study and, if required, subsequent testimony. No legal work or legal action is being performed on your behalf.

Independent Contractor Agreement — BLS Supervision Services
1.1 Nature of services
Contractor will perform services for BLS as an independent contractor. Services are "work made for hire" under U.S. copyright law. Contractor grants BLS the right to use their name, voice, signature, photograph, likeness, and biographical information in connection with the services and their promotion.1.2 Relationship of the parties
Contractor enters into this Agreement as, and shall continue to be, an independent contractor. Contractor shall not look to BLS as employer, partner, agent, or principal. Contractor is not entitled to employee benefits including worker's compensation, disability insurance, vacation, or sick pay. Contractor is responsible for all insurance, clearances, licenses, and permits necessary for conducting the services. Contractor shall not solicit or accept employment — by contract or as an employee — with any clients previously or currently working with BLS or BLS Supervision. Contractor must immediately inform BLS if there is any investigation or finding that puts their clearances or necessary licenses at risk.1.3 Compensation
Contractor shall be compensated per role as set forth below. No additional fees will be paid unless approved in advance in writing. Contractor is solely responsible for all taxes, Social Security contributions, disability insurance, unemployment taxes, and other payroll-type taxes. Contractor hereby indemnifies and holds BLS harmless from any claims arising from Contractor's failure in this section.1.4 Confidential information
All BLS Information — including trade secrets, customer lists, techniques, and proprietary information — is strictly confidential. Contractor shall not disclose BLS Information to any third party at any time during or after the term of this Agreement.1.8 Protection of client information
Contractor shall keep all client names, contact information, personal history, and medical information strictly confidential at all times during and after performance of services.3.4 Representations and warranties
Contractor warrants: (a) full authority to enter this Agreement; (b) no violation of third-party agreements; (c) work product is original and does not infringe any third-party rights. Contractor shall defend and indemnify BLS from any claims arising from breach of these representations.4.1 Term and termination
Agreement is terminable by either party at any time with or without cause. If BLS terminates, Contractor is compensated for work performed up to the time of termination. Agreement automatically terminates upon Contractor's death.5.1 Governing law
Governed by the laws of the Commonwealth of Pennsylvania. Exclusive jurisdiction and venue in the federal and state courts sitting in York County, Pennsylvania.Pay rates by role
Supervised Visitation Monitor: per assignment (set by BLS) · Therapeutic Visitation Supervisor: $100/hr — arrive 15 minutes before visit · Custody Exchange Monitor: $75 per exchange · All roles: testimony at $100/hr with $200 minimum retainer due at least 14 days before hearing. All reports must be submitted to BLS within 72 hours of the end of the visit or exchange.Application submitted successfully. Your documents and contract are under review. BLS Supervision Services will contact you within 2–3 business days regarding your approval status and first assignment.
